Personal Adviser (Leaving Care Worker) – 3 Month Contract


Location: Lincoln, Lincolnshire
Contract: Temporary (3 Months)
Pay: Bi-weekly

Summary


Join a dynamic Leaving Care Service in Lincolnshire that is expanding its team to better support Care Experienced Young People aged 16–21. This role offers a meaningful opportunity to assist young individuals in their transition to independent adulthood. As a Personal Adviser, you will provide practical guidance, emotional support, and advocacy, empowering young people with the tools and confidence needed to succeed.

Responsibilities


  • Build strong, positive relationships with young people and their support networks.
  • Develop and review Pathway Plans using Signs of Safety and Valuing Care frameworks.
  • Support young people in key areas such as:
    • Accommodation
    • Health and wellbeing
    • Education, employment, and training
    • Finances and life skills
  • Coordinate multi-agency support across housing, health, and partner services.
  • Conduct statutory face-to-face visits (local and national travel required).
  • Act as Lead Professional where appropriate.
  • Participate in a duty rota and provide support during crisis situations.

Requirements

Requirements:

  • Full UK driving licence and access to a vehicle (national travel required).
  • Experience within a local authority or council setting is highly desirable.
  • Experience supporting young people aged 16–25.
  • Background in leaving care, youth work, social care, or a similar setting.
  • Strong understanding of safeguarding, risk management, and vulnerability.
  • Experience working within multi-agency environments.
  • Proven ability to build effective, supportive relationships.

Qualifications


  • NVQ Level 3 (or equivalent) in Health & Social Care, Youth Work, or a related discipline (desirable).
