• An experienced Community Occupational Therapist is required to join an integrated locality team on a project basis.

• This role focuses on delivering high-quality occupational therapy interventions to adults in the community with complex physical and functional needs.

• The successful candidate will assess, plan, and implement interventions that promote independence, safety, and quality of life, particularly in relation to manual handling and housing adaptations.

• Provide specialist occupational therapy assessments for adults with complex needs in a community setting.

• Develop and implement personalised care plans to promote independence and reduce risks in the home environment.

• Assess for and recommend major and minor housing adaptations, including Disabled Facilities Grant (DFG) applications.

• Deliver expert advice on manual handling and safe systems of work, ensuring best practice is followed.

• Liaise with multi-disciplinary professionals including social workers, health partners, and housing teams.

• Use clinical reasoning and evidence-based practice to inform interventions and outcomes.

Support clients through the provision of specialist equipment and training in its safe use.

• Maintain up-to-date and accurate case records in line with statutory requirements and internal policies.

• Participate in regular team meetings, supervision, and continuous professional development activities.

• Contribute to service improvement and development initiatives within the team.

• Proven knowledge and practical experience in complex manual handling, optimal care techniques, and housing adaptations.

• Experience working within integrated teams and across health and social care sectors.

• Strong clinical reasoning and decision-making skills in managing complex cases.

• Excellent communication and interpersonal skills, including client advocacy and multi-agency working.

• Ability to manage a caseload independently and prioritise work effectively.

• Familiarity with relevant legislation and guidance including the Care Act, Health & Safety at Work Act, and the Equality Act.

• Competent in using case management systems and electronic recording tools.

• Minimum of 2 years post-qualification experience as an Occupational Therapist, ideally within adult social care or community settings.

• HCPC registration (Health and Care Professions Council).
