This role involves managing a caseload of complex mental health clients within the older adult service, focusing on delivering comprehensive health and social care interventions. Key responsibilities include commissioning care packages, conducting safeguarding investigations, and monitoring clients' mental states for signs of relapse and recovery. Additionally, the position requires participation in the Central AMHP team rota, providing assessment and coordination under the Mental Health Act.

Responsibilities:

Manage a caseload of complex mental health clients within the older adult service.
Provide health and social care interventions tailored to client needs.
Commission and oversee care packages for clients.
Conduct safeguarding investigations to ensure client safety.
Monitor clients' mental states to identify and address relapse and recovery.
