Job description
The Assistant Social Worker (ASW) will support vulnerable adults across the borough through the implementation of our strengths-based model of practice. This will involve:
Conducting assessments, reviews, and developing support plans that help residents and their carers maximise independence, build on their strengths, and improve quality of life.
Assisting qualified social workers with complex cases, providing hands-on support and contributing to effective case management.
Working closely with partner organisations to develop strong relationships and support innovative approaches to meeting residents’ needs and improving outcomes.
Key Responsibilities
Undertake initial and ongoing assessments of adults in need of social care support.
Develop, review, and update person-centred support plans, ensuring they reflect the resident’s strengths, preferences, and goals.
Identify risks and safeguarding concerns and contribute to safeguarding processes.
Support social work teams in safeguarding adults at risk, including participation in strategy discussions, risk assessments, and multi-agency meetings.
Monitor and report on safeguarding concerns, ensuring timely interventions in line with statutory guidance.
Build and maintain effective relationships with residents, carers, and multi-agency partners.
Support creative approaches to service delivery and contribute ideas to improve outcomes for residents.
Attend relevant meetings, forums, and training to stay informed of best practice and statutory requirements.
Maintain accurate records, case notes, and reports in accordance with organisational policies and statutory requirements.
Participate in supervision, reflective practice, and continuous professional development.
Work within the professional standards and codes of conduct for social work practice.
Requirements
Social Work qualification (CQSW, DipSW, or equivalent) with HCPC registration.
Minimum 2 years post-qualification experience in adult social care, assessment, and safeguarding.
Experience working with vulnerable adults and knowledge of safeguarding legislation and practice.
Ability to assess needs, manage risk, and contribute to support planning.
Strong interpersonal and communication skills, able to build rapport with residents and partners.
Ability to work independently and as part of a multi-disciplinary team.
Problem-solving and creative thinking to support positive outcomes.
Good organisational and time management skills, with attention to detail.
Experience in a local authority or health/social care setting.
Knowledge of the Care Act 2014 and other relevant statutory frameworks.
Familiarity with strengths-based and person-centred approaches.
