3 Month Contract With A Local Authority


Job Purpose

To provide high-quality social work services to adults with care and support needs, their carers, and families. The role involves conducting strengths-based assessments, developing personalised support plans, promoting independence, and ensuring safeguarding in line with statutory frameworks such as the Care Act 2014. Social Workers will work collaboratively with multi-disciplinary teams to improve wellbeing and enable service users to make informed choices about their support.




Key Responsibilities

  • Conduct strengths-based assessments using Care Act wellbeing principles.

  • Develop personalised support plans with clear, achievable outcomes.

  • Promote preventative activities, community resources, and telecare solutions.

  • Work collaboratively across health and social care services for smooth transitions.

  • Manage caseloads under supervision, ensuring financial and service delivery considerations.

  • Identify, assess, and respond to safeguarding concerns in line with statutory duties.

  • Lead/co-ordinate safeguarding adult cases and prepare reports for meetings and court.

  • Apply Mental Capacity Act, DoLS, and best interest assessments where appropriate.

  • Maintain accurate, comprehensive records on assessments, plans, and contacts.

  • Monitor and report progress against performance objectives.

  • Ensure all documentation is accessible to relevant stakeholders.

  • Work effectively with service users, carers, and a range of professionals.

  • Present complex information sensitively and appropriately.

  • Actively promote equality, diversity, and inclusion in practice.



Requirements

  • Recognised Social Work degree, DipSW, or CQSW.

  • Social Work England registration.

  • Enhanced DBS check.

  • Strong knowledge of Care Act 2014, Mental Capacity Act/DoLS, safeguarding procedures, and Making Safeguarding Personal.

  • Understanding of adults with long-term conditions and complex needs.

  • Experience with personalised support planning, risk assessment, and multi-agency working.

  • IT proficiency: case management systems, Microsoft Office, databases.
