1 Month Contract With A Local Authority

To provide high-quality physiotherapy across all pathways of Intermediate Care, including reablement, rapid response, and rehabilitation beds. The postholder will work within a multidisciplinary team (MDT) to support patients in achieving optimal function, independence, and recovery, both in the community and bed-based settings. The role requires experience with complex caseloads and the ability to work flexibly across multiple care pathways.

Key Responsibilities

• Assess, plan, and deliver physiotherapy interventions across all Intermediate Care pathways.
  

• Work collaboratively with MDT colleagues to develop and implement patient-centered rehabilitation plans.
  

• Support patients in regaining independence and improving functional outcomes.
  

• Monitor, record, and report on patient progress, outcomes, and service metrics.
  

• Participate in MDT meetings, case reviews, and professional development activities.
  

• Contribute to service improvement initiatives and audit activities.
  

Requirements

• Full driving licence and access to a vehicle.
  

• Registration with the Health and Care Professions Council (HCPC).
  

• Experience working in MDT and rehabilitation settings.
  

• Community physiotherapy experience.
  

• Postgraduate experience preferred due to complexity of caseload.
  

• Postgraduate qualifications or specialist training in rehabilitation, community care, or intermediate care.
  

• Experience across multiple care pathways and service settings.
