Job Purpose

To support vulnerable households to sustain their tenancies and successfully move on into appropriate long-term accommodation. The postholder will work with individuals and families at risk of homelessness, providing tailored housing-related support, coordinating multi-agency interventions, and promoting independence and stability.

Key Responsibilities

  • Provide proactive tenancy sustainment support to prevent tenancy breakdown and homelessness.

  • Assess risks to tenancies, including rent arrears, anti-social behaviour, mental health, substance misuse, domestic abuse, or financial hardship.

  • Develop and implement personalised support plans with clear outcomes.

  • Support service users to understand and comply with tenancy conditions.

  • Work closely with landlords (social and private) to resolve tenancy issues and mediate where necessary.

  • Support households to move on from temporary accommodation, supported housing, or emergency placements into suitable long-term housing.

  • Assist with property viewings, sign-ups, and move-on arrangements.

  • Support service users with practical resettlement tasks such as budgeting, utilities, benefits, and setting up a home.

  • Ensure planned, timely move-on to maximise availability of temporary accommodation.

  • Manage a caseload of clients with complex needs, prioritising cases according to risk and vulnerability.

  • Work collaboratively with internal teams and external partners including social services, health, probation, DWP, voluntary sector organisations, and support providers.

  • Attend case conferences, safeguarding meetings, and multi-agency panels as required.

  • Identify and respond appropriately to safeguarding concerns for adults and children.

  • Carry out risk assessments and ensure safe working practices, including lone working procedures.

  • Maintain professional boundaries while delivering a trauma-informed, person-centred service.

  • Maintain accurate, timely, and confidential case records in line with data protection requirements.

  • Monitor outcomes and performance targets, contributing to service reporting.

  • Ensure compliance with relevant housing legislation, policies, and procedures.

Requirements

  • Knowledge of housing legislation, homelessness prevention, and tenancy management.

  • Experience working with vulnerable people and those with complex needs.

  • Ability to assess needs, risks, and vulnerabilities and develop effective support plans.

  • Strong communication and negotiation skills, including the ability to engage hard-to-reach clients.

  • Ability to work independently and manage a caseload effectively.

  • Excellent partnership and multi-agency working skills.

  • Good IT skills and ability to maintain accurate case records.

  • Empathetic, resilient, and solution-focused approach.

  • Commitment to equality, diversity, and inclusive practice.

  • Ability to work flexibly, including occasional outreach or home visits.

  • Relevant qualification in housing, social care, or support work (e.g. CIH, NVQ, or equivalent).

  • Experience working in homelessness prevention or move-on services.

  • Knowledge of welfare benefits and budgeting support.
